You have been asked to be a part of the research described here. Participation is voluntary.

Summary of the study: The purpose of this research project is to explore people’s understanding of issues related to food waste, and their attitudes and behaviors regarding a food waste separation program on campus. We are interested in hearing from students, faculty, staff, and food service personnel. Your participation will help us understand the current limitations and future potential of an effective food waste separation program.

What to expect: You will be asked to complete an online questionnaire. The questionnaire should take about 10 minutes to complete. You may decline to answer any questions you would prefer not to answer. The questionnaire is completely anonymous.

Risks and discomforts to you if you take part in this study:

Participation in this research is voluntary. There are no physical, mental, or emotional risks associated with taking these surveys other than the minimal risks associated with normal daily activities. Please feel free to skip any question that you feel would make you identifiable or that you would prefer not to answer.

What will you receive for taking part in this study: There will be no monetary compensation for completing this questionnaire.

What will happen to the information collected in this study: Your anonymous responses will be combine with the responses collected from other members of the campus community, and analyzed to determine very generally the overall level of understanding surround food waste issues in campus, and the overall attitudes and behaviors toward a food waste separation program. The information collected will be kept confidential as much as is permitted by law. All of the data will be kept in password-protected computer files, and will be deleted within 5 years after it is collected.

What rights do you have when you take part in this study: Participation in this research is voluntary. Deciding not to take part, or to stop being a part of this research will result in no penalty, fine, or loss of benefits that you otherwise have a right to. If you decide to withdraw partway through the research study the data that has been collected from you up until that point will be discarded.
