Award Recipient FERPA Release
The Family Educational Rights and Privacy Act (FERPA) of 1974 is Federal law designed to protect the privacy aspects of a student’s educational record. It regulates the use and disclosure of personal information in educational records and permits a student to know what material is maintained in those records. This FERPA release form must be completed by the Seal of Biliteracy recipient, if over 18, or by the recipient's Parent or Guardian, if under 18. By completing this form, you allow your school to share your qualifying assessment scores and contact information to receive the State Seal of Biliteracy. This information will not be shared with any other organizations or individuals.
