Just put the SKU of the item then Qty then you will put Pick-up or Delivery.

Let say this is the item you want all you need to put is 

SKU : 31931    QTY: 50 pcs      Pick-up

Note: 
You can find SKU on the lower part of the Item name, the one with red triangle and arrow as shown in the picture below.

After you send your order form our sales person will contact you for finalizing your order, if ever you want it to be deliver the sales person will discuss the delivery charge.
