This form is to make a Safety Report to the SwitchOn Northwest Safety Director if you experience or witness something that makes you feel unsafe, including but not limited to violations of the SwitchOn Northwest Code of Conduct.

Please use this form for non-urgent safety concerns and/or to request follow-up. 

If you require immediate assistance, please reach out to any SwitchOn Northwest Event Directors or volunteers to connect you with the Safety Director.

What to expect: this form will be monitored throughout the weekend and we will follow up with you as soon as possible. While you may submit this form anonymously, please note that providing your contact information will allow the Safety Director to follow up with you with any questions and/or results of your report.
