ai_query with Databricks Foundation Model APIs is enabled by default. You only need to fill this form and enroll if you want to use ai_query with external models and custom models.

Qualifications for Accessing AI models from SQL

Databricks instance with either Databricks SQL Serverless OR Databricks SQL Pro activated
Already have a model selected for the task to be supported. Model could be a Databricks foundation model, external model, or one of the following models ready to host in Model Serving: OSS LLM model, custom LLM model, non-LLM model.
If using external models (OpenAI, etc) - already have an account with the model provider and your company approval to use it. Acknowledge that there will be data egress to the external model provider.
Supported regions for GPU serving are AWS-us-east-1, AWS-us-west-2, AWS-eu-west-1
