MHSAA pre-participation sports physical is required for all students who participate in sports. A physical examination must be completed on or after April 15, 2021 to be considered a valid physical for 2021-22. The MHSAA two-page physical form can be found at MHSAA.com.

Students who do not have a physical on file with the office by the first day will not be allowed to participate until their physical is on file. All physicals can be turned into the main office.

NOTE: No athlete will be denied the opportunity to participate due to financial need. Please contact your athletic director for confidential arrangements.
