EES Certificate Statement of Completion Application
This is an application form for matriculated graduate students to state their completion (by the end of the present quarter) of all requirements for the graduate certificate in Education, Equity, and Society. This form can be completed during enrollment for [name of EES Colloquium course] for two credits, which is the capstone project for the certificate. If you are submitting this form during your final quarter, you must submit it before the end of Week 3 of the quarter. You must have completed the "EES Certificate Statement of Intention Application" form prior to this form (and in fact prior to enrollment in [name of EES Colloquium course] for two credits. Which courses have you completed with a grade above 2.8 to meet the 16 credit requirement for the EES certificate. Please include the course number, instructor name, quarter and year of completion. Please also note that a maximum of 6 credits can count toward both your current degree program and the certificate. Toward this end, please identify which 6 credits you are double counting for your degree and the EES certificate. The other 10 credits must be beyond your degree credit requirements. (All courses must come from the approved EES certificate list - refer to the EES website to find this list. Any course not on this list must be submitted to Dr. Jondou Chen,
for EES faculty to review and approve.)
