High School Transcript Request Form
This form is used to submit information for ordering a Parent-Generated Transcript.

If you have a high school student in CHASC, please note that the annual transcript is provided at the end of every year and you should submit the course plan in the fall to begin the process. The links to all forms are in the Member Forms area of the website. Please consult the handbook and contact us at transcripts@chasc.net for more information.

You will be emailed a copy for verification before the transcript is finalized. Fees must be paid before a final copy is emailed to you.

Under the third option, parents are the administrators of the homeschool and therefore, parents sign all transcripts.
