Chemical Dependence and Psychiatric Comorbidities in Affluent Adolescents
You are being invited to participate in a research study about chemical dependence and psychiatric comorbidities in affluent adolescents. This study is being conducted by Michael Vallerie at Wheaton College, Norton, MA. There are no reprocutions if you decide to participate in this research study. There are no costs for participating in the study. The information you provide will help develop a greater knowledge of chemical dependence and psychiatric comorbidities in affluent adolescents. The questionnaire will take approximately 4 minutes to complete. The information collected may not benefit you directly, but the information learned in this study should provide more general benefits.

This survey is anonymous. Do not write your name on the survey. No one will be able to identify you or your answers, and no one will know whether or not you participated in the study. All data will be pooled, so no individuals data will be represented by itself in the event of a presentation.
