PLEASE NOTE THAT FILLING OUT THIS FORM DOES NOT GUARANTEE YOUR SPONSORSHIP. Invoices will be sent via PayPal to the email address you provide and must be paid upon receipt. If payment is not received within 5 days, the sponsorship will not be confirmed. Your swag materials are due no later than March 1, 2021.
