If you are a new client and looking to hire Urban Paw you’ll need to fill out the following form for our initial meet & greet where we’ll review your information and meet your pet. During our initial consultation (typically 30-45 minutes) we will address any questions you may have about us. We ask many questions in order to best protect your pets and other clients’ pets. Detailed information enables us to use extra care and to take any necessary preventative measures while providing for every pet in our care.
