Maintenance Issue Reporting
This form is for the purpose of reporting maintenance issues (common area and clubhouse) that need to be addressed. These are issues that involve the common areas such as low hanging limbs, light bulbs out, trees encroaching on roofs, gutters, fences, etc. It would include clubhouse issues such as damage on tennis courts, furniture issues, equipment, etc. It is NOT to report DCCR violations. Those should be reported to the ACC or BOD. For any emergency call 911.

Information needed in form is for residents to report maintenance issues. Information reported by a resident will only be available to the Roseheart BOD and webmaster.
