Outstanding Fathers Nomination Form All Nominated Outstanding Fathers Will Be Recognized at our Annual Reach 4 Peace Picnic Event on Sunday, 6/17/18 from 12:00noon - 4pm Terms and Conditions The deadline for Nomination submission is May 15th 2018 The SOL Father of the Year would meet one or more of the following criteria:Demonstrates an outstanding commitment to raising his children. Is an example of an outstanding father. Is an inspiration to others or role-model as a father. As a father is working to further his family goals and strengths. Contributes to the development and well-being of children in the community. Provides significant leadership to others. Be a father or in a fathering role Nominees may include natural fathers, adoptive fathers, step fathers, foster fathers and grandfathers raising their grandchildren (i.e. acting as their father). The children must be GHA Residents. Be of excellent character • Good reputation • No potentially comprising problems related to drug or alcohol abuse, child abuse or criminal convictions. Rules for Nomination • A father can be nominated by someone else who knows him. • A father can be nominated multiple times (by different nominators) Availability All Outstanding Fathers will need to attend the Annual Reach 4 Peace Picnic.
