This form is to be used when applying for the Lions Club Scholarship.

You must submit a separate application for each and every scholarship for which you are applying. Make sure that your responses fit the scholarship requirements. Some scholarships require additional items such as transcripts, essays, etc. You are responsible for submitting all required items to the Pupil Services Office by the scholarship deadline.

After you complete and submit this form, your responses will be emailed to you as an editable Google Doc. Feel free to add or correct information in your responses, but do not change any of the application questions or headings. When you are satisfied with your responses, you are responsible for printing your application and submitting it to the Pupil Services Office.
