Tryout Release of Liability and Assumption of Risk

By allowing my child to participate in Everest Volleyball Club tryouts, I acknowledge and agree to the following:

1. Voluntary Participation

I understand that participation in volleyball tryouts is voluntary and involves physical activity, including running, jumping, diving, and rapid movements.

2. Assumption of Risk

I recognize that volleyball tryouts carry inherent risks of injury, including but not limited to sprains, strains, falls, collisions, and other athletic-related injuries. I voluntarily assume all risks associated with participation.

3. Release of Liability

I release and hold harmless Everest Volleyball Club, its coaches, staff, volunteers, and facility partners from any liability, claims, or demands arising from injury or accident during tryouts.

4. Medical Authorization

I authorize coaches or staff to administer basic first aid and to seek emergency medical care if necessary. I understand that I am responsible for any medical expenses incurred.

5. Parent/Guardian Consent

I certify that I am the parent or legal guardian of the participant and that I have read, understood and agree to this Release of Liability.
