If you need a replacement parent name tag, please complete the form below. All parents of new ESJ students do not need to order name tags, and will automatically receive them either at New Student Orientation in August or in January if transferring midyear.
Parents are encouraged to wear their ESJ parent name tag any time they're on one of ESJ's campuses or attending an ESJ event, from athletic contests and arts performances to volunteer opportunities and fundraisers.
When the name tag is delivered, it will be given to your oldest child from their advisor (grades 6 - 12) or homeroom teacher (age 1 - grade 5).
