Courier Request
Use this form to request a courier pick up for samples to be shipped to the state lab for testing.

Please request a courier 24-48 hours in advance of your desired pick up.

The couriers do not pick up at private residences.
  • The CDPHE courier service should not be used to transport bacterial water samples. Bacterial water samples are required to be received at the lab within 26 hours of collection for processing.

On the day of your scheduled pick up, please call the courier line within one hour of the expected pick up time between 8 a.m.-4 p.m. (303-691-4040) if your pick up is missed. (Please check map for times the courier is in the area).
