You can appoint a named advocate by completing this authorisation form. The purpose of an advocate is for that person(s) to act on your behalf in relation to your studies with OCA.

Before completing this form, please ensure you have the permission of your advocate to share their information, including personal information and contact details. Any information provided will be held by OCA in line with our Data Protection and Confidentiality Policy.

Your advocate should:-

agree with you in advance clear guidelines in relation to confidentiality;
raise any issues as directed by you, relating to your studies in a timely manner;
only raise issues as directed by you;
only speak to OCA as agreed by you;
accurately represent your concern;
accurately report OCA's response back to you;
remain objective at all times;
treat all OCA staff with respect;
conform to OCA's policies, specifically the Terms and Conditions.
