This form can be completed by a StARS student or client, a community member, a StARS staff member, or a member of another service provider.

StARS encourages feedback and/or complaints about our services and the conduct of our staff. You are NOT required to include your name or contact information when providing feedback or making complaints; however, please note that our ability to address anonymous feedback and/or complaints may be limited.

StARS will acknowledge receipt of your complaint within 7 days after its submission. We will endeavor to process the complaint within 28 days; however, in some cases, we may require more time. If we require more time, we will inform you. If you have questions or concerns after you have submitted a complaint, please contact StARS at 25759451 or
