Complete the following form as accurately as possible (all information will be kept confidential, and used only for music department Planning.)
Agree to the CHCI Extracurricular contract by checking the appropriate boxes at the end of the form.
Login to your School Day account to sign the Consent For Ongoing Off-Campus Activities Form and pay the $40 Student Activity fee. Go to www.chc.wrdsb.ca
to login to your School Day account, or to register for a School Day account. The form and fee are accessible to all students registered at Cameron Heights. Please let me know if you have any trouble finding the form.
