By completing the fields below, I acknowledge that:

1. I have read the Participant Information Form.

2. I have read the information provided and any questions I have asked have been answered to my satisfaction. I agree to participate in this research, realising that I may withdraw at any time without reason and without prejudice.

3. I understand that all identifiable information that I provide is treated as confidential and will not be released by the investigator in any form that may identify me unless I have consented to this. The only exception to this principle of confidentiality is if this information is required by law to be released.

4. I have been advised as to what data are being collected, the purpose for collecting that data and what will be done with the data upon completion of the research. I agree that research data gathered for the study may be published, provided my name or other identifiable information is not used.

5. I agree that the community pharmacy that I own and/or work at, will participate as a site for disseminating osteoporosis-related information as part of the 12 month trial. This includes offering the information to eligible patients and, with their consent, collecting their mobile numbers to send the information via SMS using Fred Dispense.

6. I understand that I can withdraw consent at any time by removing the application from the App Bar in the pharmacy’s Fred Dispense Plus software or by contacting neha.sharma@healthily.com.au
