Please use this form to inform me that an assignment is or will be late. I will accept pretty much any request to submit late work as long as this form is submitted and as long as you give me enough time before the end of the semester to get it graded for you. If you end up missing multiple assignments, it may be easier to submit just ONE form with all of the assignments listed. Lastly, don't feel like you have to answer all of these questions.
