If you intend for your child to continue in the Southern Adirondack PTECH Program for the next school year, please complete this form and return to your school counselor by the deadline below. If you wish to drop your student from the PTECH program, please complete the linked drop form below.

STEP ONE:
Once you have completed the application and permission form, notify your high school counselor by APRIL 15. You will need your SCHOOL COUNSELOR'S NAME, EMAIL AND PHONE # for this form.

Once complete, CLICK SUBMIT at the end of the form. At that point, the student, parent, and counselor will receive a notification that the student portion of the application has been submitted.

STEP TWO:
Student must contact their School Counselor and ask them to complete the COUNSELOR APPLICATION located at: PTECH Counselor Application - Year 2

!! APRIL 15 = Student portion of the application must be completed by APRIL 15.

!! APRIL 30 = Counselor applications must be completed and submitted electronically via the Google Form by APRIL 30
