TSP Phase 2: Final Participant Survey
This is an evaluation form to collect and gather feedback on the Textbook Success Program, particularly your experience in Phase 2 (last 9 months). Your responses will remain confidential, viewable only by Rebus staff. Responses can remain anonymous should you choose. The survey should not take longer than 15 minutes to complete.

Your input will help us:
1. Gather information on the value and impact of the program;
2. Improve the program's efficacy — be it relating to the curriculum, structure, tools and delivery;
3. Determine your interest in being a TSP facilitator and/or collaborating on program revisions.

We believe it's important for potential program participants to hear directly from alumni and graduates, so public reviews about the program may be shared on the Rebus Community website and in marketing materials about the program. As stated above, other feedback provided in the survey will remain confidential.
