This form is required for all John Martinson Honors Program students in their third or fourth year at SIUE who do are not required to meet with an honors advisor.

Completion of this form allows your Honors Meeting (HM) hold to be removed. For students who need to register for HONS 499, it also serves as your course prioritization request.

Please note that this form does not replace your meeting with your major advisor. You are still responsible for completing that advising process separately.

You must complete and submit the form in order to have your hold released. You will receive a copy of your responses as confirmation of submission.
