NOTE: Schools make reservations and arrangements with artist/vendor; BOCES contracts with vendors and provides payments. This process often takes 6-8 weeks so you must fill out this form well ahead of the program date.

CANCELLATIONS: It is the responsibility of the school to contact the provider if the program does not take place. The BOCES will pay artist or vendor unless notified by the school district that the program/event was cancelled or rescheduled.
