Informed Consent Agreement *
I hereby acknowledge that certain RISKS OF INJURY are inherent to participation in any activity. This includes the activity in participating in the Perth Children's Water Festival held at Wildwood Conservation Area. Types of injuries may be minor or serious and may result from one’s own actions, or the actions or inactions of others, or a combination of both. Notwithstanding this acknowledgement, I hereby release the Upper Thames River Conservation Authority and all other partners and their employees, servants or agents while acting within the scope of their duties. I declare having read and understood the above Waiver of Liability and Assumption of Risk in its entirety and hereby consent to participate acknowledging all of the foregoing. I understand that I will not receive any remuneration, salary, wage or payment or any employee benefit whatsoever, or be covered by Worker’s Compensation Benefits.
