NOMINATION CRITERIA – All nominees submitted for Honorary Life Membership consideration must meet these requirements:
1. Rendered outstanding service to the Association
2. Nominated to the OFMA Board of Directors for recommendation
3. Once approved by the Board, the nominee must be approved by a majority vote of the members present and voting at the Annual Meeting

APPLICATION DEADLINE –11:59 p.m. on May 6, 2022.

AWARD DATE – Nominees selected for Life or Honorary Membership will be presented with his / her award at the next OFMA Annual Meeting and Banquet following the nomination.
