Agree & Permission
I hereby certify that I am the adult parent or guardian of child listed above, a minor child under the age of eighteen years, and I consent to his/her participation in Clementine Arts LLC ART CAMP at the Field Workshop located at 217 NE 97th Street, Miami Shores, Florida.
I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of serious bodily injury, property loss or damage) of (1) said minor child’s participation in recreational activities at the Clementine Arts Workshops. I recognize my responsibility to ensure that said minor child participates in print club activities while I am overseeing and responsible for my child.I understand that Clementine Arts LLC shall have no responsibility to pay for medical treatment and related costs if said minor child is injured.
The minor’s parent(s) or guardian(s) understand that all activities are VOLUNTARY and that the minor does not have to participate. It is understood that the Newark Print shop does NOT provide any insurance coverage for the minor’s person or property; and minor’s parent(s) or guardian(s) acknowledge that they are responsible for the minor’s safety and for the protection of the minor’s property.
In exchange for allowing the minor to participate in these Clementine Arts LLC activities, the minor by and through the undersigned, agrees to release from liability, agree to indemnify, and hold harmless the Clementine Arts LLC, and any Clementine Arts LLC staff acting within the scope of their duties, for any injury to the minor’s person or damage to the minor’s property.
I agree, personally and on behalf of the minor child named above, to assume all the risks and responsibilities surrounding my minor child’s use of the Clementine Arts. To the fullest extent allowed by law, I hold harmless and agree to indemnify Clementine Arts LLC, its officers, directors, faculty, staff, volunteers, employees and agents, from and against any present or future claim, cause of action, loss or liability for injury to person or property, which said minor child may suffer or for which said minor child may be liable to any other person, related to said minor child’s participation in recreational activities at the Clementine Arts, regardless of fault.
Permission to Use Photograph:
I grant to Clementine Arts LLC, its representatives and employees the right to take photographs of my minor child and in connection with the above-identified subject. I agree that Clementine Arts LLC may use such photographs of my minor child with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and web content.
