Eagle Chamber of Commerce 2024 Awards Nomination Form

Welcome to the annual Eagle Chamber of Commerce Member Awards nomination process! We are excited to invite you to participate in this special opportunity to recognize outstanding Members of the Chamber. Please take a few moments to complete the nomination form below and help us honor the businesses and individuals who have made a lasting impact. Your nomination plays a vital role in celebrating excellence and supporting the continued success of our Chamber members. Thank you for your contribution!  

Please use this nomination form to nominate an individual, business or non-profit organization separately. For example, use this form to nominate an individual and then submit the form. Then, submit a separate nomination form for a Business of the Year nomination and a separate nomination form for the Non-Profit of the Year. 

Deadline to submit this nomination form is November 15, 2024. In the interest of fairness to all Nominees, Nominators and organizations, the Eagle Chamber will strictly adhere to this deadline. Incomplete or incorrect nomination forms will not be considered. 

The following are stipulations with regards to nominations and Nominees of the Eagle Chamber of Commerce 2023 Awards.

  1. Only Members of the Eagle Chamber are eligible to submit nominations and only Eagle Chamber Members are eligible to be nominated. 

  2. Recognition period: These awards recognize accomplishments in 2024; thus, ensure only to reference actions from January 1- November 15, 2024.

  3. It is permissible to self-nominate.

    • In this case, the Nominee and Nominator on this form are the same.

  4. Members of the Awards Committee (judges), their immediate family, those living with them, the organization for which they work, and their co-workers are ineligible for nomination. 

  5. Eagle Chamber staff and Board Members are ineligible for nomination.

  6. Only one nomination form per Nominee will be considered by the Awards Committee.

    • Submission of additional nomination forms for the same Nominee will produce no bonus. Thus, it would be best if you worked with the Nominee to produce and submit the best single nomination form possible.
