The New York Aquarium Sumer Camp Release Form 2011
In order for your child to enroll in Aquatic Adventures Summer Camp 2011 at The New York Aquarium, you must complete this form.

COMPLETED forms must be received by the New York Aquarium Education Department by the first day of your child's registered camp session.

Be sure to list one or two guardians, aside from you, that are permitted to pick up your child at the end of a camp day. Deviations from these written instructions will not be allowed unless the parent/guardian calls the Registrar at (718) 265-3457 with specific information prior to dismissal time. If such a call has not been received then the child will wait in the Education Department office until the appropriate family member can be reached to verify the identity of the designated person who has come to collect the child.

If only one designated person will be listed, please ignore the request for the name of the second designated person.
