Parents / Guardians,
In an effort to stream line the sign up process, I designed this form for all the information needed for each student. This form replaces the Emergency/Permission Card that needed to be filled out each individual trip. You will still need to fill out a Payment Slip telling me what your student(s) need for each trip (Lift Ticket, Bus ride, Rental, etc). This data will be with me on all Ski Trips in case of emergency. Please take your time filling this out, this information is very important if your child happens to get injured on one of the trips! If you have more than one student attending Ski Trips, please fill this form out for each student. If through the course of the Ski Club season, the information you enter needs to be updated, just re-enter all of the information again and I will update my records.

Please note - If an injury is severe enough to transport your child to a hospital, the protocol at Devils Head or Tyrol is to bring them to the nearest facility. The nearest two hospitals are in Baraboo and Portage. If the injury is very severe the doctors at those hospitals will decide to transport or not to a Madison hospital. Please note which Hospital in Madison you prefer.
