Utica University Field Site Evaluation
In order to evaluate the pre-service teacher's fieldwork as part of their overall course performance, please complete this form. Consider the responsibilities that were agreed to at the beginning of the fieldwork experience and recorded on the fieldwork schedule contract. Your responses on this form will be considered when assigning the grade. We encourage teachers to discuss evaluations with the student conducting fieldwork; they are permitted to see the evaluation.  

TIP: An easy way to share the evaluation with the student conducting fieldwork is to forward them the email receipt that you will receive after submitting this form.
