The Lakehead Orillia Flu Shot Clinic is open to all Lakehead students, faculty, and staff. We have one clinic scheduled for Wednesday November 1st at Heritage Place, and one scheduled for Monday November 6th at the Wellness Centre on campus at 500 University Ave. If you are unable to attend either clinic date, you can get your flu shot at no cost at your local pharmacy.

Complete this form to select a timeslot to receive your flu shot. Please print and complete this consent form and bring it with you to your appointment. If you don't complete the consent form in advance, please arrive to your appointment 10 minutes earlier to complete a paper copy.
