This form is intended for former Beachwood High School students only.

To send copies of your Beachwood High School transcript, complete all sections of this form. Use a separate form for each recipient. Please allow 5-10 business days for processing.

Please contact the Admissions Office to ensure that the school accepts electronic (email) copies prior to submitting your request for such. If the school will receive an electronic (email) transcript submission then be sure to include the best email address to prevent processing delays. 

For any requests that need to be mailed, please verify the appropriate mailing address, prior to submission, to avoid returned mail.
  
Note that official transcripts cannot be faxed. 

Attention: If you are in the process of withdrawing a student from the high school and want your records sent to the new school please have the school contact khughley@beachwoodschools.org directly. If you are a current senior your counselor can send your transcript to the school(s) of your choosing; please be in communication with them about that process.
