Welcome to the VAULT Festival 2018 LATES Technical Form. We'll use the information you enter here as the basis for our preparations for your event so please make sure you're happy with it before you submit.

Please note, you can only submit this form ONCE.
Please make a copy of your responses BEFORE submitting.
Failure to submit your information on time may result in your requirements not being met, or the cancellation of your event.
Please make sure to read the LATES Production Pack and the LATES Tech Spec BEFORE submitting.

Requirements
Please explain as clearly as possible - especially if you have any non-standard requirements.
Lighting Requirements *
Please try to be as specific as possible. The festival provides a standard rig (containing LEDs, Moving Lights and a Stage Wash). Anything you require on top of this, you will need to provide (we may be able to provide out of our spares stock on a first come, first served basis). A lighting technician will be available on your get in to rig and programme lights and will stay for the event itself too. Lighting plans and tech specs for the LATES can be found on www.vaultfestival.com/resources.

Please note, there is A £40 + vat cleaning charge (per event) if your event uses these. This is due to the extended cleaning that is required post-event.

Have you uploaded the proof of your Public Liability Insurance? *
A minimum of £5 million Public Liability Insurance cover is required for your event to take place. Please upload your proof of PLI here: https://www.dropbox.com/request/ngcYB1lHe0lVPqSJJCde. Please title it "Event Name - PLI". If your event contains Aerial / Fire / Pyros / Special FX / Lasers or other high risk activities, we will also require proof of PLI cover from the respective performers/riggers/operators of these activities.
Have you uploaded your Health & Safety documentation, including any relevant method statements & risk assessments? *
A Risk Assessment is the minimum we require for your event to take place. A template and guidance can be found on the Resources page of our website. If your event contains Aerial / Fire / Pyros / Special FX / Lasers or other high risk activities, we will also require Method Statements for these activities.

CREW REQUIREMENTS
A Sound Technician and Lighting Technician are provided for your load in and event as standard. You can book additional tech crew members directly through us. Please note that charges apply. Please select the crew members you'd like to hire for the periods during which you'd like their assistance. Please give more info in the "Other" box if you need to.

Hospitality Rider
We can provide drinks for your artists / performers (beer, wine, spirits, mixers, softs, water) at cost price. All hospitality rider will be recharged to you. Please enquire for prices - please give an idea of what you would require below. Please note, that anyone employed to assist on the derig must be sober/not intoxicated.
