Diocesan Youth Day Retreat Registration Form

Items to note: 
  • We will not be providing food for attendees, so you are required to bring their own lunches and snacks with you. Please avoid bringing any foods that contain tree nuts as this is a common allergy.
  • For students under 18 years old who are attending the retreat, parents or legal guardians will be required to fill out a Youth Registration, Medical Release, and Permission Combined Form. You will be emailed a copy after registering.
  • For those students over the age of 18 who are attending the retreat, they will be required to fill out an Adult Medical Waiver, Liability & Photo Release Agreement. You will be emailed a copy after registering.
  • Unless otherwise stated, the retreats will end 15 minutes prior to the start of the Vigil Mass for the participating parish, so please plan to pick them up ahead of Mass (even if you plan on attending).
