KCDC Fall Concert - Student Workshop
Thank you for registering your dancer to attend Kansas City Dance Collective's student workshop! 

Student Workshop, October 24th @ 4:00-5:30pm at City Stage Theater in Union Station
For Dancers Age 9-18

During their time with us dancers will:
-Talk with KCDC Company Members about what goes into producing a show and how choreography is made
-Learn excerpts from our show on the stage of City Stage Theater in Union Station
-Have a guided small group breakout time where they work with each other, under the direction of a company member, to practice their own choreographic skills
-Have an included student ticket to stay and watch the 7pm performance on 10/24/26
(Note: 1 student ticket is included in the purchase of workshop for the Saturday, 7pm Performance. 
Anyone attending the performance with the student will need to purchase their own ticket for admission to the show.) 

REGISTRATION DEADLINE: Friday, October 11th, 2026
(In order to guarantee your dancer will have their included t-shirt ready for them on the date of the workshop, please fill out this registration form by Noon on Friday 10/9/26. If you fill out your form after this date, we will still order your t-shirt but it might not be available until after the workshop date. Thank you!)

Cost to Participate: $60 Donation to Kansas City Dance Collective due by October 24th, 2026

If Participant is Under 16 year old, they will need an adult contact on site once theater doors open for seating. Please provide the name and number of the contact we are to make sure they are in the care of by 6:30pm. If you need KCDC to provide someone on-site for your dancer to have as their on-site contact, please let us know. There will be a sign out sheet and we will not release a dancer until we've signed them out.

PARTICIPATION WAIVER AND RELEASE OF LIABILITY
In consideration of the risk of injury while participating in dance and as consideration for the right to participate in the Activity, I hereby, for my child, knowingly and voluntarily enter into this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of his/her participation in the Activity, and do hereby release and forever discharge Kansas City Dance Collective, their affiliates, managers, members, agents, attorneys, staff, volunteers, and representatives for any physical or psychological injury that he/she may suffer as a direct result of his/her participation in the aforementioned Activity, including traveling to and from an event related to this Activity. I AM VOLUNTARILY ALLOWING MY CHILD TO PARTICIPATE IN THE AFOREMENTIONED ACTIVITY ENTIRELY AT HIS/HER OWN RISK. I AM AWARE OF THE RISKS ASSOCIATED WITH TRAVELING TO AND FROM AS WELL AS PARTICIPATING IN THIS ACTIVITY, WHICH MAY INCLUDE, BUT ARE NOT LIMITED TO, PHYSICAL OR PSYCHOLOGICAL INJURY, PAIN, SUFFERING, AND ILLNESS. I UNDERSTAND THAT THESE INJURIES OR OUTCOMES MAY ARISE FROM MY OWN OR OTHERS’ NEGLIGENCE, CONDITIONS RELATED TO TRAVEL, OR THE CONDITION OF THE ACTIVITY LOCATION(S).
