Meal Order Form - Beginning December 1
Meals will once again be provided while we are in Full Virtual. Meal service will begin on Tuesday, December 1, 2020. Meal service will be provided as follows:

Option 1 - Just stop by and pick up meals every Tuesday and Thursday from 11:00 - 1:00 at the Middle School Entrance. You will receive breakfasts and lunches for multiple days. If you want this option, stop now, you do not need to complete this form.

Option 2 - Pre-Order meals for the week and pick up a 5-Day Meal Box on Monday's from 11:00 - 1:00 in the Rear of the High School. You MUST pre-order by completing this form for this option.

You only need to complete this form if you are selecting the 5-day box pick-up. You DO NOT need to complete this form if you will use the Tuesday/Thursday walk-up pick-up at the Middle School.

You only need to complete this form once. Your order will remain and be prepared for every pick-up.
