AV D&D Player Waiting List Survey
This survey is for players who would like to join the AV D&D program in the upcoming term and who are unable to find a space in a group for their age group at a time that suits them.

How this works:
- Please complete the fields below to tell us your age group, availability, and additional information.
- We will compare your availability with other players and our DMs and DMAs.
- If enough players within the same age group are available at the same time, we will contact you with more information.
- If we are unable to schedule an additional group this term, we may still be able to consider your availability for future terms.

- Any additional group/s would depend on DM / DMA availability, and would be held online via Zoom only.
- If an additional group is offered, that group would be required to meet the minimum number of registered players in order to run.

Please note:
- This is a survey to gather availability for a potential additional group.
- Completing this form does not constitute a guarantee of a space in the AV D&D program.
- Priority will be given to players who are not yet registered for a D&D group at all in the relevant term.
- Priority will also be given to current AV members (if you are a current member, please provide the email address linked to your AV membership).


If your player is aged under 18 years, please complete this survey from their perspective.
If you have multiple D&D players in your family, please complete this survey once for each person who would like to join the program to ensure we have an accurate representation of additional player enquiries.
