Technology Request Form

A Technology Request Form should be completed for all new requests that cost more than $1,000 or impact more than five individuals. The form can be completed by a committee, department or individual.

The completed Technology Request Form will be reviewed by the Technology Steering Committee to ensure alignment with college strategic goals, confirm that existing alternatives do not exist and to provide a means to advise the Dean about
high cost/high impact requests. The TSC will provide feedback and recommend next steps. The review process will also validate the possible need for the creation of a subcommittee or task force before work begins.

** Please note **
This form is not required for instructional software used by faculty or in classrooms/labs. Instructional requests should be directed to the appropriate department head for review and submittal.
