2025 Scholarship
We’re glad you are applying for this scholarship. We can help you pay for tuition or school fees up to $1,000. Our mission is to provide the education necessary for youth who have either lost their parents or live in foster care or group homes, to CHOOSE empowering choices, so they can LIVE a life of independence, and then LEAD their family and community. This scholarship will be awarded to a teenager aged 18 or younger who is pursuing education. The scholarship winner must be someone who is living in foster care or a group home, or someone who has lost their biological parents.  No one related to the Choose Live Lead board members are eligible. Information may need to be verified before scholarship is awarded. By completing this application, you are stating you understand this. The deadline to complete this application is November 15, 2025. We will notify you of the decision by December 1, 2025. This form needs to be filled out by the applicant who will receive the scholarship.
