AWC 2025 Feedback Form

Use this form to give feedback for every Session you attend. This is helpful for the AWC and its speakers. You can fill out this form anonmously. However by providing a valid name and email address, you will be entered into the giveaways.

Each submission will count as one (1) entry into the giveaways, but only one form per presentation can be submitted. Multiple enteries increases your odds of winning. However, we endeavor to avoid awarding multiple prizes to the same winner. With that said, we have a generous amount of giveaways this year!

To enter the giveaways, all forms must be completed by Sunday, June 29th at 11:59 pm EDT, but the form will stay open until the end August 2025 for anyone wanting to leave feedback.
