Thank you for taking the time to fill in our Community Wellbeing Survey.  

The survey is aimed at people living in Lubbesthorpe and will help Lubbesthorpe Alive strengthen and shape what we do.  It will also enable us to connect with other agencies and funding bodies who can contribute to the wellbeing of our community.   Some of the questions are designed around areas we have identified as becoming relevant to this growing community.

The survey is designed to be anonymous.  However if you'd like further information or help around any of the issues addressed then please either leave your details at the bottom of this questionnaire or email lubbesthorpealive@gmail.com.    This will remain confidential and we won't share anything without your permission.

If you'd like the chance to win one of our Aldi vouchers then please leave a contact phone number, this won't be shared with anyone and will be removed from our database once the vouchers have been distributed.
