2019 Summer Band Camp Registration Forms
Northport Tiger Marching Band Camp August 6-10, 2019

Dear Parent or Guardian,

Your young musician will be attending Marching Band Camp 2018 at SUNY Farmingdale. A third letter explaining packing, travel details and student room assignments will be handed out at the required parents meeting(s). The experiences gained through student participation at band camp are a major factor in the outstanding quality of our performing ensembles. We hope your child has a wonderful musical experience at camp this summer and we look forward to working with them throughout the year! The parent demonstration at Farmingdale will be August 10, 2019 at 1:00 pm. All parents and families are welcome to attend.

**Part of this form includes your most recent tetanus shot date - please have that date ACCESSIBLE when filling out this form to alleviate multiple submissions**

FINANCES: The BASIC FEE is $410. The NET TOTAL IS DUE AND PAYABLE ON OR BEFORE FRIDAY JUNE 26, 2019. If you need to confirm the credit from fundraising, please contact our band camp head chaperone, Ms Marie Michalopoulos at:

LOCATION & FACILITIES: SUNY Farmingdale will be the site for this summer's and clinic. The music building, practice fields, recreational facilities, and dormitories, are all available to us! The fine for any LOST KEY is $250.00. (We will provide lanyards, however, to prevent keys from being lost.)

PLEASE NOTE ANY DAMAGE TO THE COLLEGE FACILITY WILL BE ASSESSED DIRECTLY TO THE PARENT OF THE STUDENT INVOLVED. In the unlikely event of situations of this nature occurring, the decision of the Music Department will be final.

INSTRUMENTS: Please make sure instruments are in excellent playing condition now, as there will be no repair service at Farmingdale. Also, students will need to bring an adequate supply of reeds, valve oil, etc.
