This form is those who are wanting to take part and help the running of the STORM event, which will
take place on the 15th August 2021 as part of Fringe By tHe Sea in North Berwick. If you are unavailable on that date, or do not intend to take part in the event, please do not complete this form.

You must be aged 16 or over on the 15th August 2021 in order to be part of the STORM TEAM.

GDPR - the information collected in the form shall be used by Vision Mechanics Ltd solely for the
purposes of managing the event safety at STORM on the 15th August and for report writing
(anonymous information only). Information may be shared with 3rd parties involved in coordinating
and responding to emergency protocols for the event.

Following the event we will ask for you for some feedback and ask if you want to join a mailing list,
after that all details will be permanently removed from Vision Mechanics and any 3rd party
information holder.
