Purpose and Goal

NKY Health is committed to providing a safe workplace for all employees.

To that end, all employees and supplemental staff receive annual training about the proper use, care, and disposal of Personal Protective Equipment (PPE).

The goal of this form is to confirm that the training was completed and to affirm understanding of the content and related policies.
