Parent Transition Survey

Completing this survey will help teachers, counselors, and administrators to better understand your and your child’s expectations for the future and will provide vital information that can lead to successful transition planning.

This survey is designed to help determine what type of experiences and education the student will need to
prepare for life after graduation. It can be used to develop a long range plan (or a transition plan) which will be discussed at the student’s yearly ARD meeting. The student and the parents should fill this out together.

Not all of the sections or choices in this survey may be directly relevant to your child, but please complete those sections and choices that best reflect your concerns and thoughts about adult life for your child.
