READ CAREFULLY:

Please fill out this form anytime you have an absence outside of the school day, even if has been excused through the school. Please be sure to fill out the correct date for the absence. This will help us maintain accurate records and in turn, ensure that we enter your grade correctly.

Please note that filling out this form DOES NOT automatically excuse an absence. Just as when you miss school, you must provide specific details for us to determine whether or not an absence is excused. Absences will be excused in accordance with FISD policy and the FHS Band handbook.
