Bus Pass Application 25/26 School Year
Hello PHUSD Families!  

Below is information regarding transportation for the 2025/26 school year.  Please read carefully and then fill out this application if your student needs transportation.  This application is for up to three students. If you have more than three students applying, you will need to submit another application.

If this is your students first time in our District (or did not have their school picture taken the previous school year), you will be required to submit a picture of your student for their bus pass with the District Office. A photo is required before the student will receive their bus pass.

Bus Pass Price Per Student:
Annual: $330
Biannual: $165
Annual AM or PM Only: $165
Biannual AM or PM Only: $83

You will be notified once your students bus pass has been completed either by ParentSquare announcements or email.

Acknowledgment of Bus Rules & Procedures  
Please review the Bus Rules & Procedures with your student.  Bus Pass Rules and FAQ are posted on Parent Square.  Do you (parent or guardian) and your student(s) agree to all rules and procedures concerning bus conduct and that bus transportation privileges may be suspended for rules violation.  

If you select "No" you cannot receive a bus pass.
